The OhioMHAS Office of Prevention Services has developed a survey to gather feedback on the Center of Excellence for Behavioral Heath Prevention and Promotion to help inform future efforts. This brief survey will only take about 10 minutes to complete. Participation is voluntary and responses are anonymous. No personally identifiable information will be associated with responses. The deadline to complete the survey is April 27.

Our primary care provider is your partner in care and is here to provide information, listen to your needs, and help you make wise choices to improve and maintain your health.
For many mental health conditions, combining therapy with medication may be the most effective way to get results.
Case Management focuses on your ability to succeed in the community and accessing needed services. We help improve your functionality in school, work, family, and integration within the community.
We offer clinical assessments for all age groups. These assessments are needed for admission to our programs. We also provide them as an independent service to other agencies.
